1. Defining Self Care There is a wide range of self-care support interventions. These have varying levels of intensity, differences in content, and a range of theoretical underpinnings (from didactic education to the use of behavioural counselling techniques and theories of behaviour change). 2. Providing Self Care The organisation will endeavour to provide maximum support from the pharmacy to enable people to care for themselves or their families. There is a range of topics ranging from long-term conditions to minor ailments on the website in the well-being section that aims to provide information and advice in support of patient’s conditions. This information is also linked to the NHS Choices website for up-to-date information and support and also if the patient would like more in-depth scientific knowledge. Self-care our pharmacy intends to provide our patients include: information-giving addressing motivations and barriers to change teaching coping strategies designing action plans dealing with emotional sequelae of illness ongoing monitoring/support engaging family and social support 3. Opportunistic self-care The support for self-care will also be provided opportunistically if a patient or carer telephones the pharmacy for advice. A note will be made of any advice given to patients whether online or over the phone if deemed necessary. This service intertwines of course with signposting and public health promotion.
